It seems to happen when I shut of my computer without closing iTunes first? I feel like it began when I downloaded iCloud. So finally tonight I got so fed up I deleted iTunes and every piece of Apple software then downloaded iTunes again. I noticed that I was using 32bit and the new version is 64bit. The annoying part is I have to add my music folder to my library again, and then make copies of all my playlists from my phone, then add them to the library, then delete all the music on my phone by setting it to manually managed, then I can uncheck that and add the selected playlists again.
